Elderly and Disabled Waiver Program (EDWP)
CCSP/SOURCE Elderly and Disabled Waiver Program (EDWP) provides skilled nursing and personal care support to Medicaid adult members 21 years and older. Care4All provides quality care to members through health education, teaching, medication administration, nutritional counseling, and maintenance care to prevent regression. The agency assist individuals recovering from acute illnesses. These services are expected to maintain or increase the functioning capacity of the client while in the comfort of their own home.
Skilled Nursing Service
This includes, but is not limited to; monitoring, supervising, and providing the care of those individuals whose health status involves complex observations, initial evaluation, development of care plan and care coordination.
Personal Care
Personal Care tasks include; assistance with bathing, toileting, grooming, shaving, dental care, dressing and eating (emphasizing proper nutrition), home management, bill management, housekeeping, ambulation and transfer, and medically related activities (including taking vital signs).
Companion Sitting
Companion or Sitter includes: transport and escort services, meal preparation and serving, and household tasks essential to cleanliness and safety.
Companion or sitter services are provided to the elderly, physically challenged and convalescing individuals. These services can either be provided at home or as escort services while on outings with family members. In-home services include light meal preparations and serving and household tasks essential to cleanliness and safety.
