A Licensed Private Home Health Care (PHC) Agency   

Care4All Community Care, Inc. is a Licensed Private Home Care Agency (PHC) providing quality in-home Nursing and Personal Assistance, in the larger Atlanta Metro Area, by Certified staff who assist individuals fulfill their basic activities of daily living. Georgia Pediatric Program (GAPP) provides skilled nursing care and personal care support to medically fragile children under the age of 21 who are Medicaid eligible. CCSP/SOURCE Elderly and Disabled Waiver Program (EDWP) provides skilled nursing services and personal care support to Medicaid eligible adult members 21 years and older.

1. Nursing Services: Including, but not limited to, Medication Administration, Physical Assessments, Wound/ Injury Care, Urinary Catheter, Charting, Trach, Vent, G-Tube etc. 

2. Personal Care Services:  Personal Care tasks includes: Assistance with bathing, toileting, grooming, shaving, dental care, dressing, and eating (this may include proper nutrition), home management, housekeeping, ambulation and transfer, and medically related activities, including the taking of vital signs in conjunction with the above tasks, and/or assistance with bill management.

3. Companion Sitting Services: Companion or Sitter means the following tasks which are provided to the elderly, physically challenged or convalescing individuals; transport and escort services; meal preparation and serving; and household tasks essential to cleanliness and safety.

We Serve: Elderly, Children, Convalescents, Mentally Challenged, Developmentally Challenged, Physically Challenged, Recovering from Surgery Veterans, Private Pay, Long Term and Short - Term Assistance in the privacy of your own home.
